Why Does My Back Keep Going Out?

Every week someone tells me their back "went out" doing something ridiculous. Reaching for a coffee mug. A sneeze. Picking up a sock, not even the laundry basket, the sock. And they're always a little embarrassed, as if their spine betrayed them over nothing.

The sock didn't do it. A back that goes out over a sock was ready to go out for months, sometimes years. The sneeze is just the bill coming due.

What "going out" actually is

Your spine doesn't slip out of place like a drawer coming off its rails. What actually happens: a joint that's been moving poorly for a long time finally irritates the tissue around it enough that everything clamps down. Muscles guard, movement locks, and standing up straight becomes a negotiation. It's protective. Your body is throwing a circuit breaker.

The pattern I watch for is the spacing. First it's once every couple of years, then yearly, then it's every few months and the episodes last longer. That trajectory is the real problem, because each episode isn't a random accident. It's the same underlying pattern getting louder.

Why it keeps happening

Because the trigger gets treated and the pattern doesn't. Rest and ibuprofen calm the episode down, the guarding releases, you feel normal, and the joint that started it all goes right back to moving badly. Nothing actually changed, so the next episode is already scheduled. Nobody knows the date yet, that's all.
